- Authorization to Use and Disclose Protected Health Information 11-12 (required for each student)(Spanish Version)
- Medical Policy & Authorization for the Administration of Medication (if applicable)
- Physician Request for Self-Administration of Asthma Medications (if applicable, 3 pages)
